Puppy Buyer Questionnaire and Contract

On this page you will find our puppy questionnaire and contract.  Please note that if you are interested in a puppy from us, don't fill out the contract yet.  Just fill out the questionnaire.  You may view the contract, however we don't request you send this to us, until you are promised a puppy from one of our litters. 

We request that you please print off the questionnaire and fill it out, sign it, and return it through the mail (send to: 82002 Highway 11  Burwell, NE 68823).  We keep these on file for all of our new puppy homes.

Also, once you are promised a puppy from us, print off two sets of the contract.  Please sign both of them and return both to us here at Perfect Peace.  We will then sign both copies and return one with your puppy packet.
