Puppy Buyer Questionnaire and Contract

On this page you will find our puppy questionnaire and contract.

We request that you please print off the questionnaire and fill it out, sign it, and return it through the mail (send to:   74331 321st Ave    Imperial, NE 69033  )(or email/fax).  We keep these on file for all of our new puppy homes.

Also, once you are promised a puppy from us, print off two sets of the contract.  Please sign both of them and return both to us here at Perfect Peace.  We will then sign both copies and return one with your puppy packet.
